Based on what I found inside - a light coating of ink on everything - I think the repeated cleaning produced an ink "fog" inside and coated everything, even the sensor. Just ran the alignment a second time, both pages printed and it was successful.

Sure enough it had some kind of semi transparent film/grime on it. The printer is working, but the printed output is missing black or color ink, or the printed page is blank or has very little ink.
